Issue:
At first can not access the questionnaire questions from Front Desk > Reception 
> Client Details > Client Activities > Questionnaire. [add a new record] enter 
a date, no questions are displayed. [save] Questions now are displayed. Have to 
[edit the current record] to enter the answers to the Questionnaire. Need to 
have the questions displayed when first add a new record.
