    a.  TRICARE Prime Point-of-Service Option Fact Sheet:


Published: 10/21/2011

Description: TRICARE Prime Point-of-Service Option is a two-page fact sheet that

provides information about the point-of-service (POS)

option for beneficiaries enrolled in TRICARE Prime and TRICARE Prime Remote for

Active Duty Family Members. The fact sheet also covers the

POS option for beneficiaries enrolled in TRICARE Overseas Program (TOP) Prime

and TOP Prime Remote. Information about cost-shares, deductibles, and

explanation of benefits is included as well as contact information. 


    b.  Identity Theft Recovery Fact Sheet:


Published: 10/19/2011

Description: Identity Theft Recovery is a two-page fact sheet that provides

information about how beneficiaries can safeguard personal information. The fact

sheet gives step-by-step instructions on what to do in the case of identity

theft. Additional resources, including consumer reporting companies, identity

theft resources, privacy information, and contacts for information and

assistance are also included in the fact sheet. 


    c.  TRICARE Eligibility Fact Sheet:

Description: TRICARE Eligibility is a six-page fact sheet that provides general

TRICARE eligibility information for sponsors and their family members.

Information about eligibility by sponsor type, other beneficiary categories, and

special circumstances for TRICARE eligibility is included as well as contacts

for further information and assistance.  


    d.  TRICARE Catastrophic Cap Fact Sheet : 


Published: 10/19/2011 

Description: TRICARE Catastrophic Cap is a one-page fact sheet that provides

information about the catastrophic cap: the maximum out-of-pocket amount a

TRICARE beneficiary will pay each fiscal year. Contacts for further information

and assistance have also been included. 


    e.  TRICARE Prime Enrollment Fees Fact Sheet: 


Published: 9/29/2011 

Description: TRICARE Prime Enrollment Fees Fact Sheet describes changes to

TRICARE Prime enrollment fees for fiscal year 2012 (October 1, 2011-September

30, 2012). Information about exceptions for survivors and medically retired

service members is also included.
