Following up on the retreatment conversation from our CoP meeting-
First, I recently spoke with a pharmacist from Masshealth who indicated that
they consider treatment-experienced to be anyone who has received more than a
week of a previous regimen (so someone receiving <1 week would still be
considered tx-naive). Neither he nor I are aware of any data or research to
support this, but we agreed that that felt reasonable. Is anyone aware of any
data to support lengthening that timeframe at all?
These tx-experienced individuals, if not cured with their initial regimen, are
equivalent to treatment failures and should be treated with a regimen
appropriate for hx of tx failure (in MA that is likely going to be Sof/Vel/Vox
(Vosevi) every time).
And second, I just noticed that hcvguidelines.org has updated their
tx-experienced section to include Sof/Vel/Vox (Vosevi) failures. Aimee, I think
this what you were asking about and appears to be in keeping with David Nunes'
guidance.
It looks like these recs are based on very few cases and expert guidance, but
at least good to know that no one knows what to do.

Thank you to everyone who was able to make time last evening to meet and talk
about HCV treatment issues and experiences across Massachusetts!
We had 8-10 participants and a lot of great discussion, and it was honestly
just really nice to feel like we could lift our heads up out of the COVID
milieu for a moment and reconnect around this type of care.
I am attaching the agenda slides and have added notes within them from our
conversation, including:
- verbiage to use for PAs when you have indeterminate fibrosis and
can’t access further fibrosis assessment testing
- Accessing HCV tx during acute infection window
- David Nunes’ recommendations on treatment options for Vosevi
failures (currently options and data aren’t great but there are some options if
it is urgent to retreat now)
- Initial reactions and thoughts on current PA issues and how a
removal of PAs might work
I wanted to make you sure you are also aware of a few upcoming opportunities
The EndHepCMA Coalition will have its first meeting in a year next Thursday

And finally, we discussed plans to continue our CoP meetings on a quarterly
basis, with a tentative next meeting on June 2nd. As the date gets closer we
will confirm that date and send along more info!
In the meantime, please use this listserv to ask case questions (deidentified,
of course), troubleshoot logistical and PA navigation issues, or share training
opportunities, etc.
In that vein, I am sharing the notification that I received this am that INHSU
2021 abstract and scholarship submissions are open from now until 4/25/21.

them out- they are dedicated to the health of substance users and engage and
support PWUDs to participate and present at their conferences alongside
academic researchers and community providers.
