Though if there are control characters (like line feeds), would that cause a problem? Would that make the standard PAX block reader code think that it has finished reading the current key=value line and treat the rest of the string as a new key=value entry? Or does the length field cover that? I guess you'd have to try it with existing Tar code to see how it is really done.
I have checked both gnutar and bsdtar - they handle multiline string comments seamlessly. Looks like the length field has high priority.
