You could have two icon menus for document-
oriented applications:

[(app-icon)|(document-icon)|Edit|...]

If desired, and only when it fits the window:
[(icon)StyledEdit |{icon}{filename}|Edit|...]

(This could also allow drag&dropping the document
to save it, sort of RiscOS(?)'ish, if people want that
nowadays. Personally I'm growing less and less fond
of mouse work.)
