[GradeQuick] Skill Fields

We use Elementary Skills Short Portrait report card form, and use the skill
fields for things like Completes assignments, Knows basic facts, Spells
correctly in written work. Since we did not modify the standard GradeQuick
report card for elementary schools, we also put Effort Grades and Year
Average in skill fields.
