Seems you never composed. Not all MIDI data streams originate from a human 
performance.

It also seems you are not up on the current technology. Nowadays plenty of MIDI 
data streams are sent over USB or Firewire. Those have significantly higher 
bandwidth 
and lower latency than the original physical layer of MIDI.

Some MIDI data streams never go over any physical layer at all but are instead 
rendered by (possibly
non-realtime) software synthesizers or software samplers.
