is UST really nanoseconds?  or is it better defined as "some host-based 
monotonic signal with resolution eter than or eaqual to" some value, such as
1ms?

The only thing this doesn't handle well is edge-detection, which can be done
with modulo arithmetic, if you don't mind every plugin doing it.
