[gmpi] MIDI: Host knowledge reqs

The current idea seems to be that the host would implement parameter knowledge in terms of the dest parameter path. Assume a plug could also give the host a number to represent each dest parameter. Would host knowledge still work if it tracked parameters by the number? Yes, right?

I'm going offline for a couple hours, I think I have an idea that finds a common coding between MIDI and pure GMPI while transcending MIDI's limits (opcode set, address space, operand datatypes & range). And still works with host knowledge.
