[foxboro] green death happens

While I believe my fate is sealed, is there any way to determine which modu=
le in a formerly fault tolerant pair has succumed to green death?

How do other users avoid this jam (ROM address marked on the front)?

How prevalent is green death?  Histories with HW/SW data might be helpful.
