 Kashif,

Possibly /opt is not getting un-mounted when you are in single user
  Mode running a fsck.
Loop back mounts exist between opt and usr. These are defined in =

  /etc/vfstab.

/opt gets un-mounted as part of the shutdown process to single user.

Analyze your /var/adm/messages files for clues to current/past
problems.

 Scott

Kashif,

Are there any messages that come up after the mount:warning:line for
"Segmentation Fault" error? mount:warning errors are usually related
to problems in /etc/vfstab.
