The problem is probably not associated with AMs, as ours was not. It is 
strictly a matter of the DMs not being closed out properly.

It's possible that the DM sessions are not being closed out properly
on exit and that is what is causing your orphan list problem. How do
your users exit? Do you have an autologout system setup? Alex's
suggestion will be very informative to determine the root cause here.
