We have had a similar issue with this card (FBM241c) just recently.
Although our symptoms were: no SMC alarms nor any red LED, but rather we had
stuck channels (no indication of failure) until a forced reboot by pulling the
card out.
Not sure how to check operational status on this sorry Dirk,

Hi,
This morning our maint dept contacted me because a number of valves could not
be operated any more. On DCS everything looked normal, it wasn't until I
checked the I/O field enclosure I noticed this card was bad, red led on.
We didn't get a system alarm.
The card was online and on scan in the system management.
The FBM is configured as an ECB8, so with PLB. The PLB looked completely
normal, all closed limit CIN's were present. If I opened a valve, the CO on the
PLB was activated, but the card didn't process this.
Switching the card off/online didn't reset the card. Al though system
management showed status online & on scan, It remained bad with red led.
I was forced to pull the card to force a reset. The card came up with a green
led after this.
What surprises me is that it can go bad without generating a system alarm.
Anything I can check or change to make sure the card goes off scan on bad
operational status?
