If the internal DNS server is a 2003 machine, take a look at this:
It fixed problems i had with outgoing mail

Hi I have a problem right now where the outgoing mail is getting queued but it
never sends out.  The incomming mail is fine as well as all local mail.
I am running 2003 and 2000 in a AD 2000 domain.  EX servers are useing an
external dns for dns lookups and the server itself is useing internal dns
servers (the AD).  it seems that when I added the external dns to the smtp
virtual server it started sending mail.. but now the mail is getting queued up
