Check Q 321652.  A simple registry fix that cured this very same problem
for me.

Hi all,

Please advice on this error while connecting to exchange using outlook
in the corporate mode. We have exchange 2000 and outlook 2000. All but 2
staff use Outlook in Corporate Mode. 

When users attempted access this morning, A dialog asking for password
appears repeated. The following error is reported

Your logon information was incorrect. Check your username and domain,
then type your password again. If your account is new or if
administrator requested a password change you need to click Change
Password then logon with your new password

A somewhat related issue occurs with ISA which repeated ask for
authentication. When the ISA server is set to use Basic Authentication
to authenticate Outgoing web requests, it works yet it fails with
integrated Authentication.

Please note access via POP3 and OWA (using basic access) works just
fine. My Exchange is on its own box, ISA on its own box and a separate
box running Active Directory Services
