Does this apply even after SP4 and the post SP4 hot fixes?!

Happen to me before, done everything but the relaying still took place.
End up the culprit is a registry key.  For some reason the setting set in
the
IMS does not write the correct registry value into the key Relayflags.
I have to manually modify the registry key before everything works.

I have followed the instructions for every method described to prevent SPAM
and relaying.
 
At my company has 30+ domains that is goes by and we accept inbound mail for
all of those.
However, we still send THOUSANDS of email spam messages every night.
 
I have changed the passwords, disabled unused accounts and read all I can on
the subject.
 
So how in the WORLD am I to stop this crap?!
