Crazy SMTP question for you experts!

My boss needs 2 email addresses. one for his personal (first@xxxxxxxxxx)
and one for work (firstlast@xxxxxxxxxx). His secratary has permission to
check his work email and send all emails in his regard. He does not want
any one to see his reply email as first@xxxxxxxxxx no matter who he sends
emails to. he wants them all to see firstlast@xxxxxxxxxx how do i make his
account accept mail only for first@xxxxxxxxxx but show a reply of
firstlast@xxxxxxxxxxx I can not add the smtp email firstlast@xxxxxxxxxx in
his AD account b/c it tells me that the email is already being used.
Besides, he doesn't want to ever check his firstlast@xxxxxxxxxx email b/c
only his secratary checks that. Please help.
