See if the delegate also appears in the managers Send on behalf setting in AD. Had a similar issue where a user had a stale delegate and would not remove. The delegate did not even have a mailbox anymore. Saw that the delegate also had send on behalf of for the managers properties. Once I removed it, the delegate was removed in Outlook.
