You could also use GFI Mail Security if you like... It provides multiple
Virus scanning engines that scan for virus' in the IIS SMTP queues. Multiple
Virus scanning engines = better protection.
Also, this product provides customizable HTML threat, Trojan, Attachment
Checking, E-mail Exploit checking, Compressed attachment scanning,
keyword/subject filtering and a wealth of other tools.
I have been using the product for over a year now and it has worked great...
By the way our domain averages over 4 million incoming e-mails per month
(spread across two MX servers) and with all of this 'SMTP Scanning' we have no
delays in mail delivery. If anything it has increased delivery times by
keeping the crap out.

You can look into a product called ORF, but doing so requires the use of event
sinks, which is resource intensive. There is a product that a version to work
with SMTP VS in IIS is being developed, but there is no current time frame
available for beta.
