AFAIK the application-specific icons should not be theme-dependent. Looking at the code, I'd expect the "missing" icon to be substituted if: 1. The custom icon file can't be accessed (bad name, bad path, bad permission) 2. That file can't be processed (corrupted file, missing png-processing infrastructure) Type 2 seems less likely, especially if the custom icons are all visible in a "select-icon" dialog. Such dialog does not use quite the same process for creating the displayed icons, so there's a chance for a bug, too. Hence my previous suggestions about examining all possible causes of a type-1 problem. After that, I'd have to suggest testing with some customised files with extra error messages, but that may not be feasible in your circumstances.
