[duxuser] heading not appearing in contents page

Hi
I'm using the contents generator to make a table of contents in a file
with the british no capitals template.
For some reason, the heading "note to braille edition" will not appear
in the contents even though I have used the h1 style. I can't find
anything different in what I've done this time to what I've done in
the past when it's worked.
Also, each time I use the contents generator with this type of british
braille template, I get the prompt: "this document does not have the
rp codes in the header in order for dbt to generate a table of
contents. Press y for dbt to add them". Why don't I get this prompt
when I use the contents generator in other files?
