No and I doubt you'll find it in any other Wiki system. If a change
needs to be aproved the greatest advantage of a wiki is removed:
chnaging things quickly. You should really think again if you want this
feature (it's a typical management wish), with a restriction like this
you will probably not see the acceptance and usage of your wiki you are
expecting.

You can have namespace templates wich prefill pages with certain data
which can aid people in adhering to certain standards but it is probably
not what you want.

Again this seems not to be a good application of a Wiki. Wikis are great
to manage unstructured data (texts) what you want is very structured and
is probably better covered in a custom database application.

However there are Wikis which provide a framework for custom
applications embedded into the Wiki (I think twiki or jotspot have this)
but I cannot say anything about it.
