This isn't entirely true, SMF 1.1RC3 has a captcha on registration that
includes audio for accessibility.

Well audio captcha.. seems like new idea with old captcha pain. Spamers use a new mechanism to workaround captchas - they include captcha pictures in any other page of user interest and users itself fill in values. Spamer then uses users input to login/post to completely different site. I already met this and it makes captcha pretty inefective.

This is my "2cents" for captcha discussion.
