I'll just copy pastroklos here instead of typing.

There are two prerequisites

   - Kill the Crystal Golem <http://darksouls.wikidot.com/crystal-golem> in
   a cave near the Darkroot Basin Hydra to free Dusk of
Oolacile<http://darksouls.wikidot.com/dusk-of-oolacile>.
   Answer "yes" when she asks whether you want her help or not.
   - Place the Lordvessel <http://darksouls.wikidot.com/lordvessel> in
the Firelink
   Altar <http://darksouls.wikidot.com/firelink-altar>.

These two conditions will allow the Crystal
Golem<http://darksouls.wikidot.com/crystal-golem>in The
Duke's Archives <http://darksouls.wikidot.com/the-duke-s-archives> to
add a Broken
Pendant <http://darksouls.wikidot.com/broken-pendant> to your inventory
after it dies. You can kill the Crystal Golem before saving her, but it
will not give you the Broken Pendant. As long as the Broken Pendant is in
your inventory, it doesn't matter whether Dusk is alive or not. You can
have her dead and still access the Additional Content.
Then, follow these steps

   1. Go to The Duke's
Archives<http://darksouls.wikidot.com/the-duke-s-archives>.
   There will be a Crystal
Golem<http://darksouls.wikidot.com/crystal-golem>near the elevator
after the first bonfire.
   2. Kill this Crystal Golem <http://darksouls.wikidot.com/crystal-golem>.
   A Broken Pendant <http://darksouls.wikidot.com/broken-pendant> will be
   added to your inventory automatically.
   3. Go back to where you fought Dusk's Golden Crystal
Golem<http://darksouls.wikidot.com/golden-crystal-golem>in Darkroot
   Basin <http://darksouls.wikidot.com/darkroot-basin>, a portal will
   appear there.
   4. Examine this portal. A cutscene will trigger. After the cutscene, you
   will be in Sanctuary Garden<http://darksouls.wikidot.com/sanctuary-garden>
