The U.S. Fire Administration has received notice of the following
firefighter fatality:

Name: Derrick T. Harvey
Rank: Lieutenant
Age: 45
Gender: Male
Status: Career
Years of Service: 14
Date of Incident: 01/08/2004
Time of Incident: 0626hrs
Date of Death: 01/15/2004                        
Fire Department Name: Philadelphia Fire Department        
Fire Department Address: Fire Administration Bldg., 240 West Spring
Garden
St., Philadelphia, PA 19123, Attn: Commissioner Harold B. Hairston

Cause of Death: Lieutenant Harvey was seriously injured and burned when
he
fell through the first floor of a residential structure and was left
hanging from the joists over the basement where the fire originated.
Other
companies initiated rescue efforts and extricated Lt. Harvey from the row
house. He was transported to Albert Einstein Medical Center via PFD Medic
Unit, then transferred to Temple University Hospital Burn Unit where he
died from injuries received. The Philadelphia Fire Department Fire
Marshal
has determined the cause of the fire to be combustibles too close to a
fixed heater.
