Hi robert. What about the runways that are more than 4000 feet above sea level?does the restriction calculat 5000 ft above the runway or 5000 ft above the sealevel?

Hello Hadi, The restriction is enforced on jetliners - not on G/A aircraft. It is impossible for IYP (or even a real pilot) to land a jetliner on a runway with an altitude 100 feet above sea level, being 18,000 feet on base. It's completely insane. If you are 12 miles from the airport, and you're not down to an altitude of less than 5,000 feet, you'll never make a landing in a B738. There's no need to execute a visual approach 40-miles from the airport unless you're flying VFR. A commercial aircraft never flies VFR.... they always fly IFR. Thus, it is the responsibility of ATC to get you onto base at a suitable altitude.I hope this clarifies the situation and reasoning behind the changes. In some parts of the world, the altitude of runway even reaches 4000 feet (take tehran for example), and the mountains and buildings around the area makes it impossible to descend to 5000 feet first then execute the visual approach. taking a come fly with me session for example, i even had to execute the visual approach when i was 40 miles out from the airport, so while we were approaching the entry point i had to gradually decrease my altitude. the new ristriction makes this situation impossible, and you can't literally land without crossing the airport for several time to get into the required altitude.
