It can happen randomly too, although you can go a few days without any
mishap.
Now wouldn’t you think that a simnple shutdown & restart of VoiceOver would
solve the issue till next time? (It has worked for other Braille issues in
the past.) After all, restarting the phone means most people have to enter
their passcode again.

Subject: [braillesupport] IOS 14.2 bug.
Hi there.
I have tried the chord 4 5 6 command on an Iphone running ios 14.2 and a
focus 40 braille display. It carried out the function as it should in that
it brought the focus to the last option on the screen, i.e to the music app
on the home screen.
On further investigation however it appears that the bug is only present if
some of the apps have been moved around or a new app loaded. A simple
reboot of the device to fix this bug should suffice.
