Yes, there's that, too.
I got a book, not even a textbook, just a book for the general reader, which
has a very small amount of simple math in it. But since the math is all in
images, I can't even read it. A volunteer produced version, properly
proofed, would have made sure that would have been included.

The problem with a lot of the PQ stuff, particularly textbooks is that all
the charts are images. In some books, half the book is images! Thus, the
need to scan and proofread!

Well I don't have the patience to scan; takes too long; and I used to proof
but that got too bogged down too with rules and regs, so I quit.
Now that we are getting so much publisher quality stuff, I don't see the
need to proof any more.

Sharon, I believe that this question is answered in the proofer's
manual, however, I will say that if you place three contiguous stars
and hit 'enter', Word, at least, will create a line of stars which is
not what you want here. You want to put spaces between each star.
