Check with a good hearing aid dealer about headphones that are built into 
custom ear molds. I just did this and while the solution isn't cheap ($200), 
it's perfectly doable. Any good hearing aid dealer should be able to check 
with his/her suppliers to find out if any of them can provide these. Such 
earphones are used by on-air television and radio personalities, the Secret 
Service, etc.

My apologies for covering old ground, but I need some really good in-the-ear
headphones which will have excellent fidelity and really block out the
surrounding sound.  I am at the gym a lot with the bookport and the noisy
atmosphere is making it hard to enjoy books using the buds I am currently
using.  Any advice is welcome.  If you don't want to clutter the list, write
me at donter@xxxxxxxxxxx although some new folks may be interested in this
topic.  I know some folks were using some in-ear models by Sennheiser but I
have forgotten which ones.
