I think Deborah is correct. Just make them consistent. Use lower case Roman 
numerals consistently where they appear in the text, such as in preliminary 
page numbers, and use upper case ones consistently where those appear, such as 
in chapter titles and medical terminology and wherever else. The main point is 
not to mix the two, as in Ii for instance, where you have an upper case I 
followed by a lower case i.
