Darren why was it rejected? I still have the book and can probably correct
whateverer is wrong (I assume it's not the Index. If it's missing something
like copyright info or title page you can email the page (that's allowable) and
I can fix it and sent it back to you; trhen you can resubmit with the proper
pages. If the page breaks and/or numbers are missing,, that's another matter.
