Doug found a way that worked well in a book he worked on a little while
back.  I believe it was using several em dashes together, surrounded by a
space at either end to create the blanks.  Bet he'll tell you if you ask.

My girls have always loved "create your own story" books where you fill in
the blanks, but I was wondering if a book like that would be a good fit for
Bookshare, and if so, how to handle the blanks in a book like this.  The
title is Mrs. Dinglehoffer's Stew and other Tall Stories.
