I've caught Fine Reader doing it occasionally, too. Usually, though, it's
two words run together because Fine Reader didn't recognize a hyphen.

The cause? In a word, Omnipage! That's its favorite error, along with the
fact that it has never heard of the em-dash. It gives pretty decent results
in most cases, but for books I always use Fine reader.
