Pam, You want to create a fake header that mimics the other real headers in
the book. For example, if the real headers contain a page number and
author's name, that's what you want to use. Same with page number and title.
What I do is put my fake header on the very top line of the page, then skip
a line, then have the chapter heading. I leave the blank line between the
chapter heading and the text.
