Sounds like you are a perfectionist, like I am.
I think it's so beautiful when I can go to the table of contents, find a
chapter and go directly to it based on corresponding page numbers. That
tells me that a reader can treat the book just as any sighted person does,
and doesn't have to read the book from beginning to end to find what they
want.

I had something similar with a book I am working on. There were page
breaks, but they often did not agree with page numbers, which could be in
the middle of the page. So first I made page breaks, but I am afraid I
left some blank pages, because I wanted the Table of Contents to agree
with the page numbers. Then I erased the headers and footers.
If you read the volunteer's manual, you will see that the program that
uploads supposedly erases headers and footers for you, but I have heard
it does not do a great job.
I am afraid this is not very helpful, but at least you know someone else
went through the same thing.

The book seems to have several small scanning errors (the comes out as
