What I do in a case like that is not rate it adult but in the long synopsis note that it has some swearing. I also do that when a book is basically worthwhile but has profanity or swearing or non-explicit by suggestive sex or horror or torture as elements. Some people on this list prefer not to read books with things like that in them, but I don't think they necessarily merit an Adult rating. It depends on how much is there.
