Monica,

Wow!  I don't think created by and written by are the
same. I see what you mean by the gov. copyright entry.
It lists Tom Clancy and Steve Pieczenik as created by
authors.

I'd put Bill McCay as the author and your explanation
in the comments section. If you cana find him listed
as author anywhere in your book, I'd put a copy of
that page in the comments sectionm too. If you put all
the information you've put here in the comments
section, Gustavo can make a decision as to what to do.
He certainly should have enough information to accept
the book.
