You could put a note at the beginning of the book that says something like: "The print copy of this book contains no chapter headings. Chapters are indicated by starting the text 1/3 down the page. This scanned copy will indicate chapter breaks with the words "Chapter break." Or if you didn't want to put in all the explanation, you could just write "Chapter break without headings" at each such break. I'd avoid putting in chapter numbers when they don't exist in the print copy. Just my nickels' worth.
