I don't know why these commands would read something if the arrows would not, but obviously alt plus up and down arrow are commands. They read prior and next sentence. You can also use alt numpad five to read the current sentence. I'm just saying this because while everyone knows how to have jaws read by character word, line, and paragraph, an amazing number of people don't know the sentence commands.

Also when you hit, alt plus down or up arrow Jaws, at least version 6 and
above will read the material to you, even when if you use the conventional
arrow keys alone it doesn't, smile. No idea why, but was quite a surprise
the one day I did it by accident and it worked.

I sure don't have a solution to this problem, but in my experience the
problem has most to do with font size. Have you tried reducing the
unreadable title to like maybe 16-point? I'm not sure how small something
has to be before it's guaranteed readable, but since font size doesn't
survive conversion it might make sense to select the whole document and pick
a uniform font size that's small enough for JAWS to be happy.
