I think that is something that has to be revised in the manual. The name
of the publisher does not necessarily appear on the first page. It might,
but it does not necessarily do so. It does appear somewhere in the
prliminary pages though. It is most likely on the copyright page if it
does not appear on the title page.

That's why I asked. The manual also states that if the publisher isn't

the first page the proof would be rejected.

page 1?  Why would we do that?  doing so would change the pagination of

Actually the back of the title page is a treasure trove of useful
information for catalogers. Yes, it should be copied and included.
