    Actually, Openbook has that problem. It has a bad correction from com to 
corn. Unfortunately, it auto corrects before joining hyphinated words that span 
two lines. So, you'll often see cornmented or cornplicated. That's because it 
was written in print as

  The answer is that Any scanning software can cause that.  It is often a 
brightness setting problem, but odd fonts can cause it, and occasionally it 
just happens.  If it happens a lot, though, it is obviously a problem caused by 
the scanner not setting up their software very well, or having very very old 
software.
