Italics are only stripped from page numbers but not from any of the text or 
other navigation. Sorry for the confusion!

My experience is that you can usually tell from the context of the surrounding 
text if a letter is beginning. if you keep the italics in tact, Braille users 
will be able to see them right away, and those using screenereaders can check 
the font and know it's italicised and thus that they are reading a letter.
