Elizabeth, I have made that mistake too. (smile) I finally figured it out when I read the help page about submitting books. It took me awhile to figure it out. This is the kind of thing that I meant about Gustavo not realizing that his comments aren't being understood. I know I'm guilty of this too, of thinking that what I'm asking someone to do is obvious when the person has no idea of what I want. I think it's obvious because I've done it so many times before. I forget that the person I'm talking to doesn't have my background and experience. Lissi, in her gentle way, reminded me that I was doing this when she asked me about scanning engines. I'm finding that the words "obvious" and "assume" get me into a lot of trouble. (grin)

I just had a conversation with Gustavo. He says that other people understand his comments. He was surprised I did not understand the following to mean do not include PHD or MD in author's names. To his "title" means MD or PHD. He seems extremely annoyed to be questioned.
