You can leave in the headers still as bookshare still runs the books
through a header stripper before they appear on the site. Actually
most seem to prefer the print page numbers be left in the books,
although it doesn't really matter what you do there either, since the
stripper usually takes them. But actually it's best if you leave page
numbers so that the validators can make sure that no poages are
missing.
You definitely want to leave the page breaks, you just don't want to
save the books with linebreaks.
So in short, submit with page numbers and pagebreaks intact and no
linebreaks, and you should be fine.
