Bud, What you put below is the copyright page, not the title page. Fortunately B&N allows us to look inside the book and it had the title page, though Amazon did not. Copy what I've written below, add it to your validated copy in front of the copyright page--maybe with a blank page between, because there usually is one, and reupload your file. If the book isn't on your upload page, see if it's on page one and if so download that copy and trash it. If none of that works, send the title page to Carrie and see if she can insert it into the file. For the future, the title page is a page that has the title, author and in some cases like this book, a subtitle.
