I'd use the first copyright notice:

The second notice is probably due to the statement about it being
published
earlier in a limited edition, and probably applies to that edition.
Previously published in a limited edition by the Darkside Press.
In any case, the first copyright notice in this instance is the one I'd
use
for filling out the form. I wouldn't attempt to combine the two notices.
