If it's so easy for a validator to do that, given the quality of people around 
here, I'd suspect the tools not the people.  Check a book you've validated.

  How can I say this?  Well, if a submitter has a track record of submitting 
  books where page breaks are correct, page numbers are present, chapter 
  headings are protected and all the rest; and they aren't there in a 
  particular book, I'd bet that
  a validator may have contributed to that problem.
