I have been working on Dharma Vahini.  I have approved it twice, and
Madeleine keeps sending it back, asking where are various pages.
All the pages she isn't seeing are blank pages, where I inserted a page
break, a page number, and the words [blank page].  I have done exactly
this many times before, for example, when there is a blank page before a
new chapter.
This time, I deleted the page break I'd inserted, and inserted a new one
with control-enter.  I'm using Word 2003.  I searched for ^m, page break,
and found it, right where I expect it.
I checked if there were any section breaks, ^b, in the book, and there
aren't.
This is all I can think of to do.  If anyone has some other idea, I'll
give it a try.  Otherwise, I'll approve it again.  But I'm losing
confidence that it will be accepted, and I don't understand what's wrong.
