Some times the first letter in a chapter is in very large and stylized
font, more of an image and may not scan properly. In a case like that it
is usually pretty easy to figure out what the letter is. Right now I am
scanning a book that does not have that first letter problem, but the
first few words of a chapter are capitalized. This is not the actual way
the text is written. That is, I am sure that the author did not
capitalize those words in his original manuscript. It is just a kind of
eye candy for the sighted readers that a print book is intended for. So
it is certainly okay to change those words to standard case. On the
other hand, I don't see why they have to be changed either. In the book
I am scanning my screen reader reads them just like it reads lower case
words, so I am just leaving them like that for the main reason that it
is easier to leave them that way. If the proofreader chooses to change
them that is just fine.

The first few words of a chapter being in capital letters is a very common
practice. I know when I scan sometimes the first letter of the first word of
a chapter often ends up on a different line, so that could have been part of
the problem. I'll let someone else answer the rest.

Hi, everyone.
I have read through two books by Mercedes Lackey--Exile's Honor and Exile's
Valor. In going through the text it looks as if there are chunks of text
missing. Also, the first letter of the first word in the first sentence of
every chapter seems to be missing, and besides that, the first three words
in the chapters are in all caps. It honestly looks as if the books need to
be re-scanned or proofed again. Or something. I'd be happy to proof them,
but not sure where to start, since I'm not sure what to put in where stuff
is missing.
