I haven't seen anyone reply to this and I finally got to the library that
has a copy (I went to another library earlier in the week, but that was a
mistake, they didn't have the book).
Anyway, the chapter titles are indeed just numbers. They are in a large
grey box, so I am not surprised the ocr didn't catch them all the time.
Also, the pages that start chapters do not actually have the page numbers
on them.
In the copy at the library, ISBN 0-684-80042-X the page numbers for the
chapters are:

There are lots of pictures in the book with captions. I hope the ocr
separates the captions from the pictures. Often they are just the names of
the people, but there are also many jokes about the people.
